Self -Organising Support Circuits

Small teams of 8–12 staff such as support workers, nurses and peer mentors who work collaboratively to deliver support without micromanagement. These interdisciplinary teams self-organise schedules and roles, empowered to make decisions close to the client, with support from a light-touch, enabling structure.

Local Community Focus

Each team is rooted in a specific geographic or cultural community, building trusted relationships with clients, families, and local networks. By being locally embedded, they provide consistent, familiar support and tailor solutions that draw on community strengths and cultural knowledge.

Person-Centred & Holistic Support

Support is designed around each person’s goals, daily needs and broader wellbeing. Teams address clinical, social, and practical aspects of life, integrating formal care with family, friends, and community supports to build autonomy and long-term independence.
